Post Acute Care Nurse Practitioner / Physician Assistant

Optimum Post Acute Care – SNFs, Rehab Hospitals, LTACs Optimum Post Acute Care is seeking a dedicated Nurse Practitioner (Family/AGNP) or Physician Assistant (PA) to join our growing post-acute team. This role is vital to ensuring our patients receive exceptional, continuous care across skilled nursing facilities, rehabilitation hospitals, and long-term acute care hospitals (LTACs).

What You’ll Do

• Conduct thorough patient examinations and review medical histories
• Order, interpret, and follow up on diagnostic tests (labs, imaging, etc.)
• Diagnose and manage acute, chronic, and complex conditions
• Develop and implement treatment plans, including prescribing medications
• Partner closely with attending physicians, rehab specialists, and nursing staff
• Educate patients and families on conditions, medications, and preventive care
• Maintain accurate documentation in our EMR (RXNT)
• Serve as an advocate for health promotion and disease prevention within the post-acute setting

What We’re Looking For

• Proven background as a Nurse Practitioner or PA in post-acute or hospitalist settings
• Skilled in managing complex, multi-morbid patients across SNFs and LTACs
• Familiarity with healthcare regulations and patient care standards
• Strong communicator who thrives in an interdisciplinary team environment
• Patient-focused, adaptable, and motivated to continue professional growth
• Active and unrestricted Family Nurse Practitioner (FNP), AGNP, or PA license

This role provides vital oversight, guidance, and clinical leadership to ensure patients receive the highest standard of care in skilled nursing facilities (SNFs), rehabilitation hospitals, and long-term acute care hospitals (LTACs)
